Set out emergency triangles

Call or designate someone to call police and your company
Assist the injured if you are qualified
Obtain facts about the accident, including, but not limited to:
Name, address, home/work/cell phone numbers of all parties involved
Insurance information on the driver(s) and owner(s) of the other vehicles involved
Date and time of the accident
Location of accident (i.e., intersection, street address, city and state)
Description of damage and/or injuries
Make, model, color and license tag number(s) of other vehicle(s) involved
Description of accident
Name, address, home/work/cell telephone numbers of witnesses
Photograph the scene and vehicles, if it is safe to do so
Obtain name, address and badge number of investigating officer
Do not make any statement to anyone or sign anything; only provide authorities with:
Your name, address and driver's license number
Your company's name, address and operating authority number (if applicable)
Your insurance I.D. card
Cargo information/Shipping documents (if applicable)
